Case Management
Our team consists of four dedicated and State Certified Community Health Workers (CHWs) who provide comprehensive case management services to members of the Pacific Islander community. Each CHW brings cultural understanding, lived experience, and empathy that allow them to effectively engage with individuals and families.
Through one-on-one assessments, our CHWs identify individual needs, strengths, and barriers to care. They work closely with clients to develop personalized support plans, make appropriate referrals, and connect individuals to health, social, and community-based services. With specialized training and strong community partnerships, our CHWs serve as trusted advocates and a vital bridge between the Pacific Islander community and the resources needed to support overall health and well-being.
OLAGA - Opportunities to Live and Grow for Our Aiga (Family)
We believe that health is a vital form of wealth. Our Pacific Islander community faces higher risks of chronic conditions such as diabetes, heart disease, high blood pressure, and obesity. In response, we focus on providing culturally grounded health education throughout the community.
Our Community Health Workers deliver education in community spaces, at local events, and in our office, sharing information about the impact of sugar on overall health and ways to reduce sugar intake in everyday life. We also offer hands-on cooking demonstrations and healthy eating education that highlight nutritious, culturally relevant foods.
Through group education, peer support, and practical tools for managing blood sugar and improving physical activity, we create a supportive environment that honors our cultural heritage while empowering community members to make informed choices for their health and well-being.
Malaga (Journey) Through Healing
At the core of our program lies a deep devotion to supporting families as they navigate the challenging journey of grief and loss. We wholeheartedly understand the difficulties that arise along this path and it is our unwavering commitment to provide a safe and nurturing space. Here, healing can be explored with creativity and compassion, allowing families to find solace amidst the pain. We want every family to know that they are not alone in their journey. Our dedicated team stands beside them, offering unwavering support and understanding, and accompanying them every step of the way towards healing.
Ribbon of HOPE
Our Ribbon of Hope program is a compassionate source of support for individuals and families impacted by cancer. During an incredibly challenging time, we provide mutual aid and wraparound support to help ease both emotional and practical burdens.
Through this program, we assist with essential needs and financial support while also offering emotional encouragement, advocacy, and referrals to counseling and therapeutic services. Our goal is to walk alongside individuals and families with care, dignity, and understanding—offering hope, connection, and stability throughout their cancer journey.
Community Blood Drives
Every 2 seconds, someone in the U.S. needs blood. We regularly hold blood drives to help address this need to save lives and bring awareness to health issues in under-funded and marginalized communities. At each drive, health providers are available on site and in these spaces to share resources and answer questions in communities where folks might not receive regular health care check ups.
Ray of LIGHT
Each year, we support widows and grieving families as they navigate life after loss. Our Ray of Light program provides emotional support and mutual aid through tangible resources and practical tools designed to bring comfort during an incredibly difficult time.
We personally visit widows and their families to offer care, connection, and understanding, and we remain available to provide ongoing support in whatever ways are most needed. Through strong community partnerships, we help ensure families are connected to helpful resources and do not have to face grief alone.
